[摘要] 目的：分析全自动血液分析仪检测血常规的临床复检规则，评估其临床意义，提高血细胞分析结果的准确性。方法：根据国际实验血液学学会(ISLH)推荐的41条复检规则并结合本科实际情况，制定了本科血细胞分析仪的17条复检规则。对1 492个门诊、住院和健康查体标本进行全血细胞分析，同时用SP-1000i制片、染色、镜检。结果：1 492例标本，符合规则307例，复检率为20.57%(307/1 492)，真阳性率、假阳性率、真阴性率和假阳性率分别为13.06%(195/1 492)、7.51%(112/1 492)、77.08%(1 150/1 492)和2.35%(35/1 492)。结论：血细胞分析仪的复检规则有重要的临床意义，能有效地筛选出真正异常样本，提高工作效率，为临床诊治提供有价值的信息。

[Abstract] Objective: To analyze the retest rules of blood routine test in full automatic blood analyzer. Assess its clinical significance. Improve the accuracy of analysis results. Methods: Made the retest rules about 17 pieces, combined the 41 retest rules recommended by ISLH with the actual situation in our laboratory. Analyzed the 1 492 blood samples from outpations, inpations and healthy persons. At the same time, smear films and stain them in SP-1000i, and examined them in microscope. Result: In the 1 492 samples, 307 cases conformed to the rules, retesting rate of 20.57% (307/1 492). The true positive rate, false positive rate, true negative rate and false negative rate was 13.06% (195/1 492), 7.51% (112/1 492), 77.08% (1 150/1 492) and 2.35% (35/1 492). Conclusion: The retest rules in blood analyzer has important clinical significance. It can effectively filter out the ture abnormal samples, impove the work efficiency and provide the valuable information for clinical diagnosis and treatment.

血液分析仪是现阶段国内各级医院临床血液常规检测的必备仪器，而由血液分析仪、自动制片染片机组成的全自动血液分析流水线在大型医院亦逐渐普及。血液常规分析时血片的复检，近年来受到重视与关注，如何充分利用血液分析仪提供的结果及报警信息，在确保较低的假阴性率同时，减少不必要的推片复检，提高工作效率，是摆在检验工作者面前的问题。笔者根据所用的XE-2100血液分析仪的特点，结合国际实验血液学学会(ISLH)推荐的“41条复检规则”，设置了适合本实验室使用的复检规则，并对这些规则以及血细胞分析仪提供的信息与显微镜检查结果进行一致性进行评估。
