丙戊酸钠负荷剂量与奥氮平治疗双相Ⅰ型躁狂发作的随机对照研究(1)
[摘要]目的 探究丙戊酸鈉负荷剂量与奥氮平治疗双相Ⅰ型躁狂发作的效果及安全性。方法 选择2015年1月～2016年10月我院接诊的80例双相障碍躁狂发作患者，随机分为治疗组和对照组，每组40例。对照组采用奥氮平进行治疗，治疗组采用丙戊酸钠负荷剂量进行治疗。比较两组患者的治疗效果。结果 治疗组有效率为82.50%，对照组的有效率为77.50%，两组差异无统计学意义(P>0.05)。治疗后治疗组Young躁狂评定量表(YMRS)、简明精神病量表(BPRS)评分分别为(7.4±5.3)分和(8.8±3.5)分，与对照[(9.8±3.5)、(13.5±7.4)分]组比较差异有统计学意义(P<0.05)。治疗组汉密尔顿抑郁评定量表(HAMD)评分[(4.5±1.7)分]低于对照组[(4.7±2.7)分]，差异无统计学意义(P>0.05)。治疗组患者不良反应发生率为7.50%，对照组为25.00%，差异有统计学意义(P<0.05)。结论 采用丙戊酸钠负荷剂量治疗双相Ⅰ型躁狂发作患者的效果与奥氮平相当，且患者不良反应较少，值得临床推广使用。

[Abstract]Objective To investigate the efficacy and safety of loading dose of Sodium Valproate combined with Olanzapine in the treatment of bipolar type Ⅰ disorder patients with manic episode.Methods Altogether 80 cases of bipolar type Ⅰ disorder patients with manic episode who were treated in our hospital from January 2015 to October 2016 were randomly divided into treatment group and control group，with 40 cases in each group.Control group was treated with olanzapine；while，treatment group was treated with the loading dose of Sodium Valproate.The therapeutic effects of the two groups were compared.Results The effective rate of treatment group were 82.50% versus control group′s 77.50%，without statistically significant difference between the two groups (P>0.05).The postoperative YMRS (Young Manic Rating Scale) and BPRS (Brief psychiatric Rating Scale) scores of treatment group were (7.4±5.3) and (8.8±3.5) score，compared with control group [(9.8±3.5)，(13.5±7.4)scores]，with statistically significant difference (P<0.05).The HAMD (Hamilton Rating Scale for Depression) score (4.5±1.7) of treatment group was lower than that of control group (4.7±2.7)，without statistically significant difference (P>0.05).The incidence of adverse reactions of treatment group was 7.50% versus control group′s 25.00%，with statistically significant difference between the two groups (P<0.05).Conclusion The efficacy of application of the loading dose of Sodium Valproate is similar to that of Olanzapine in the treatment of bipolar type Ⅰ disorder patients with manic episode，which has less adverse reactions and is worthy of wide clinical use.
