脑膜炎患儿脑脊液酶活性及蛋白含量测定的临床意义(1)

     【摘要】目的探讨不同类型脑膜炎患儿脑脊液(CSF)中乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)、腺苷脱氨酶(ADA)活性及总蛋白含量变化的临床意义。方法应用全自动生化分析仪检测91例脑膜炎患儿和22例上呼吸道感染患儿(对照组)CSF中LHD、AST、ADA活性和总蛋白含量,并进行统计学分析。结果化脓性脑膜炎(化脑)、结核性脑膜炎(结脑)、病毒性脑膜炎(病脑)患儿CSF中LDH、AST、ADA活性和总蛋白含量较对照组明显升高(P<005),结脑组患儿CSF中ADA活性升高程度较化脑组和病脑组高(P<001),化脑组和结脑组患儿CSF中LDH、AST活性升高程度较病脑组高(P<005);CSF中总蛋白含量升高程度化脑组>结脑组>病脑组。结论CSF中酶活性及总蛋白含量的变化对不同类型脑膜炎患儿的临床诊断及疗效观察具有重要的意义。其中,CSF中LDH、AST活性升高有助于细菌性脑膜炎的诊断,ADA活性和总蛋白含量的变化有助于结脑与化脑、病脑的鉴别诊断。
